Relationship of Immunohistochemical Detection of p-53 Protein and Pathological Prognostic Factor in Colorectal Adenoma and Adenocarcinoma


Abstract
The transformation-related protein of 53,000 dalton, designated as p-53, has been detected in a range of neoplastic cell types including carcinoma of colon, breast and lung by using monoclonal antibody to p-53.
The objective of this study was to investigate the expression of p-53 in adenoma and the relationship between p-53 expression and pathological, clinical parameters such as gross type, histologic type, depth of invasion, and distant metastasis in
adenocarcinoma of colon and rectum.
Sixty-two specimens, 25 cases with adenoma and 37 cases with adenocarcinoma who underwent surgery or biopsy from October 1988 to October 1992 in Kangnam St. Mary's Hospital of Catholic University Medical College, were submitted to this study.
The paraffin-embedded tissue sections wer immunostained with monoclonal antibody for detection of p-53 protein.
Biotinylated goat anti-mouse immunoglobin and peroxidase conjugated with streptavidin were used.
@ES The results were as follows:
@EN 1. In 25 adenomas, nuclear p-53 expression was detected only in two cases : one case showed tubular adenoma with moderate atypism and the other showed tubular adenoma with severe atypism.
2. The incidence of p-53 expression in colorectal adenocarcinoma was 7 of 37 specimens. When the authors examined the histologic differentiation of adenocarcinoma, p-53 positive reaction was 1 of 10 specimens in well differentiated
adenocarcinoma,
3 of
18 specimens in moderately differentiated adenocarcinoma, 1 of 4 specimens in poorly differentiated adenocarcinoma, and 2 of 5 specimens in mucinous adenocarcinoma.
Although the expression rate was higher in the higher histological grade, the differences among them were not statistically significant.
3. When we examined the incidence of p-53 expression in line with the depth of tumor invasion, p-53 positive reaction was found 0 of 5 specimens in carcinoma restricted to the mucosal or submucosal layers, 2 of 7 specimens in carcinoma invaded
muscle
layer, 1 of 12 specimens in carcinoma invaded subserosa, 3 of 5 specimens in carcinoma invaded serosa, 1 of 8 specimens in carcinoma beyond the serosa.
Thus, p-53 expression was not affected by depth of tumor invasion.
4. p-53 positive reaction was 0 of 7 specimens in type 1(polypoid), 3 of 10 specimens in type 2 (fungating type), 4 of 19 specimens in type 3(ulcerative type) and 0 of 1 specimen in type 4 (diffusely infiltrating type). There were no differences
between p-53 expression rates and gross types.
5. The incidence of p-53 expression was 6 of 17 specimens in lymph node metastatic state and 1 of 20 specimens in non metastatic state. The difference between them was statistically significant(P=0.026).
Above results suggest that-53 protein could be used dependently as a prognostic indicator in co lorectal adenocarcinoma and reflects the metastatizing ability of tumor cells.
